Many people today are pondering exactly what to do or how to get started with planning for the future. And by future I mean, "what am I going to do to help my family out when I pass?" If you have as little as 15 minutes and access to a computer with a printer or a fax machine you can get yourself or a loved one registered for whole body donation . Fill out all information on all pages. We can not stress enough the importance of this stage of the process. Incomplete or incorrectly completed forms will only delay the process. If you have any questions or concerns about any part of the process please contact us by clicking here.
We will need several things to complete telephone interview portion of the process. All biographical information (DOB, social security, marital status, parents names, etc), medical information like any major surgeries, heart conditions, cancer and cancer treatments, etc. will be recorded and maintained unti the time of need. Our medical interview process is general in scope but only an accurate surgical history will enable us to place a whole body donor with medical projects for the greatest benefit to medical science.
There you have it. We realize this may not seem like a 15 minute process but let us assure you it is. Once your interview is complete you will only need to contact us if: you move or change your phone number, your next of kin moves or changes their phone numbers, their is a change in your medical status (you undergo a surgical procedure or become ill).
